Well - it *is* in the ntsysv list on my machine (and every other one I
have seen). However, to get it started at boot time you need to a link
in the runlevel directory. For example, in /etc/rc.d/rc3.d I have the
following entry:-

lrwxrwxrwx   1 root     root           16 Apr  8 14:40 S45pcmcia -> ../init.d/pcmcia

simply create something like this in rc3.d (or rc5.d if you are using
that as the default run level) and up it comes at start up.
