Generally speaking , I prefer hardware raid.
Does not put "any" load on cpu and hides the
disk "arrangement" from the os which
makes it easier to manage. Also, rebuilding
is run by i960 cpu on card and the system
is almost unaffected by such operations.
Rebuild with software raid has more impact
on system.

We are running megaraid 438 with lvd disks on redhat 6.0 which works
superbly IF you use a newer kernel which has the updated drivers.
We use 2.2.12.

So,
a) Works great for us with newer kernel(newer drivers)
b) Can not see how it should be? Linux raid works fine
     but booting on raid devices requires a more complex setup.
     Using rh 6.1 it is easy to set up. But mainenance
     and resque is not totally simple(for me anyway).
c) Can not see how. My opinion is no.
d) Yes, megamgr is available on driver cd
    and from ami web site.
   Additionally you can configure and maintain
   arrays at boot time using built in manager.
  Megamgr requires some libraries which
  you need to link to.
   Run: ldd megamgr to see which and
   add links to those missing.

A warning: Do not use
of the shelf(cheap) LVD enclosures with
megaraid and LVD fast scsi. I have tried five brands
and ALL of them cause megaraid to "loose" disks
temporarily(which leads to fail/rebuild).
Get professional enclosures or
use none at all if you dont need "hotswap".
Setting the card at a lower transfer rate fixes
this problem but this you dont want to do
if you want performance.
LVD and fast scsi is a bit sensative.

Backplanes are the best and if you want
separate enclosures that works you
can take a look at the ones Kingston manufactures.
