There's a mini-HOWTO which - I think - applies to your situation:
The "Hard Disk Upgrade Mini HOWTO".

The latest version can be found at the Linux Documentation Project:

but you should also have it on your drive (if you installed it) or on the
documentation CD. This HOWTO describes nicely the steps necessary to move a
machine to a new hardrive setup. I've used it myself last weekend - first
time I saw it and a I definitely learned a thing or two. :-)
