Dont be afaid of posting a question on the postfix listserv. Very well mannered and knowledge people. Thats your best bet.may want to dig thru the archives first.
It sounds like you will have to use Dynamic DNS with postfix. In postfix you will specific which domains that you will accept email from. i.e. healthclinic.com & remote01.healthclicic.com. something like that. Good luck.
