Every time I had this problem it turned out to be discriminator noise somehow coupling into the CG HI input (assuming you have a duplexed radio). Troubleshooting was easy: put a full quieting signal into the RX & see if it goes away.
A 0.1 uF to ground on CG HI took care of it.
