    After thinking about it more, I wonder if `missingInput` was really the 
right way to handle this problem.
    
    Another option (that is used in every other operator now) is to require 
that an `Alias` is the only way to produce a new Attribute in a non-leaf node 
of a query plan.  Thoughts?
