After testing multiple times, I came to realize that any new volume I
create on the IBM DS3400 storage array (FC) and assign to a host
cannot be seen properly from any host. I tried creating, deleting the
volumes multiple times, assigned to different hosts and am still
getting consistent I/O errors. Even tried assigning a new volume to a
host to see how a totally new OS install would behave. The install
failed with same I/O error messages spitting out like crazy. Time to
call IBM I guess. Never ever seen this before. The storage array has
no errors at all. Strange.
