We have narrowed our issue down to a database fragmentation issue (PK for EntityReferences was 99% fragmented). I don't have enough info yet to post a full reply, but we are beginning again to feel comfortable with our performance.
I just want to make sure that I made a post that our database performance was the culprit here. Specifically, it could be related to the way we inserted data into this particular environment.
