Well, I'm not 100% sure, but I think the problem was related to having
to clear the library and rescan everything... The other less likely
possibility was that it was fixed by changing the SB's Audio setting
from "Smart" Gain to Use Album Gain (if present). I had also tested my
Slimserver using SoftSqueeze to make sure it wasn't a player problem.
Using SoftSqueeze I got the same message: Can't open file blah, blah...
so I doubt that it was a player problem. I'm not 100% sure of which step
fix it, but here is what I did. -> 1. I used foobar2000 to add
ReplayGain Album Gain to one of my ripped CDs -> 2. Copied foobar2000
modified files to the Slimserver -> 3. I changed the player setting to
Use Album Gain (if present) and -> 4.  I Cleared the library and
rescanned everything. I think step 4. was the clincher. I just switched
player setting back to "Smart" Gain and the FLAC files are working.
Miracles do happen :-)

I'm still looking for an answer to my other question.
Is there a plug-in or a way to get EAC to automatically get the album
art? Preferably, in a one click process
