I keep my main library in Flac and then run a script to transcode the whole library to an Mp3 mirror. On my PC I usually use Winamp to listen to music files for editing, testing, etc. I find that about 1 in 100 of the Mp3 files that I'm creating will not play in Winamp. They load OK, I can read the metadata, but when I try to play them, I get no audio output and the progress bar doesn't advance. The odd thing is that I _can_ play these same files in Windows Media Player, foobar2000 and Quicktime.
