If they are .mp3s the first thing I would check is that your ID3v1 tags
are the same as your ID3v2 tags and that you aren't only updating one
set of tags.  Tag and Rename will show you the different sets of tags if
you click into the tabs across the middle of the window.

If that doesn't help, have a look at a sample file in a full featured
tagger like mp3tag which is free (right click and look at the extended
tags).  Does that give you any idea what is going on?

Also, you may want to look at this - APE tags are very prone to causing
"old" tags to sit around.
