The manual has a cmake section. Still a work in progress though

I've attempted a merge --reintegrate from branches/cmake into trunk. The
operation was uneventful, compilation of mod_rivet was OK and tests work
well. I haven't committed yet and I ask for your opinion (George
Petasis' in particular) for publishing this work.
