I think what pointed to ChildInitScript and RequestHandler in server
configuration script are wrong. Every times when you make changes, you must
reload apache. On production you may have not access to server configuration
and you will must call support.
You can make call the user-defined init script and request handler from default
script. If user maked own script/handler - load it (used fixed names and fixed
position, say, document root) if no - use default
