I'm afraid this is just the fundamental nature of Rosegarden.  Something like 
this would be easy to do with a notation-driven engine that interpreted 
symbols into MIDI at playback time, but Rosegarden is a hybrid notation 
editor and MIDI/audio sequencer.

The way to solve this is to create time for the extra long note, using 
invisible time signatures.  (This would be another great mini-tutorial demo 
if only I had time to write this with little pictures.)

1: Create a segment three bars long in 4/4 time
2: Fill all the beats in the first bar
3: Fill 7/8 of the beats in the second bar
4: Sitting in the second bar, insert a time signature change (Composition
   menu) to 9/8.  Have it start at the beginning of this bar, and make it
   invisible
5: Enter the quarter note spanning beats 8-9 in bar 2.  Place fermata.
6: Sitting in the third bar, insert an invisible time signature change back
   to 4/4
7: Go out to segment canvas and drag the tail of the segment out to complete
   bar 3.

That's the theory anyway.  In practice, I observe that this doesn't export to 
LilyPond correctly.  (Bug filed.)

I can't repeat this.

Just to stay on the same page...  I've opened bwv-1060-trumpet-duet-excerpt.rg 
from the example files.  I've selected all the segments, and opened a 
combined notation view.  I've tried all three layout modes, and they all work 
as expected.  With the caveat that the annotations look like crap in anything 
but the Linear Layout mode.
