The code is in RecentFiles::add() to do the right thing. Doesn't mean it will always succeed. There might have been something subtle that prevented it from detecting that the file names were identical. E.g. one accessed via a link and the other not? Still, it should always add the new one at the top. I would expect dupes, not failure to add to the top. That's a surprising issue.

Anyway, if you've got a solid procedure to reproduce, we can look into it.
