The only think that matters is the link from /usr/src/linux -> kernel.
If you are installing a new RTAI, you need to:
- download new linux kernel
- remove /usr/src/linux symlink
- untar the new link kernel
- untar the new rtai
- run make patch in rtai, build/install rtai
- configure/build/install your new linux kernel
