Add --host to `gem push`. Fixes #28485. [Erik Hollensbe]
This makes it much easier to push gems to private gem hosts. Fixed a
few of the `gem push` messages to be less misleading with nonstandard
hosts, and extracted all mentions of our API host into Gem.host.
