Hello, I can't figure out how to create a checkbox with uberkit :/

I am trying to create a simple checkbox like this: f.check_box("eula", "accepted", { :class => 'eula_check' }, "yes", "no") Hmmm can't get the syntax right when using the Uber Form Builder.
