Have you tried just converting the measurements? For example, the United States is about 3000 miles wide, so 3000 miles divided by 20 feet equals 150 miles to the foot. So every foot on your layout would represent 150 miles.
For example, if Iowa is 300 miles wide (and that's just a guess), then it would be 2 feet wide on your layout. Maybe I'm misunderstanding your question....is there more to it than that? No matter how you slice it, it's gonna be pretty tough in S scale....
