Sounds like you don't have SA set to start automatically. I don't have SA in front of me, but it's a checkbox in the setup options. You can tell it to start checking automatically (or to wait X seconds after start), and which hosts file to load when it starts.
When you start SA manually, do you have to click Start to get it to start checking? If so, then that's the problem.
