Ummmmm, how about NOT by schedule or other automated process.

If a human being decides a check needs to be in maintenance, then send an
alert.

What is manualy?
You can put an entry into maintenance via the interface/telnet/ssh (in v5
via an alert too).
And if you would use saWeb then the telnet option become very common, since
saWeb uses the telnet interface...

Rats.

I'm not sure if I like this option, but...
How about a generic alert if any check is manually placed in maintenance?

That would mean a huge change in the way the alerts are
checked/generated...don't think this is something that will come any soon
into the product.

What we are however thinking about is some kind of generic alerts were you
can generate an alert when a host is more then .... (minutes/hours) in a
down/maintenance state. This alert would be for all entries (or maybe per
group).

Well, after a check is reported as down, and the alerts go out, I'd like to
be able to set an additional alert to go out if someone comes in and places
the check in maintenance at the GUI front end (as opposed to if it gets
placed in maintenance by a schedule).

Not sure to understand what the alert should do...

I'd love a new alert setting:

"and when manually put in maintenance in the GUI after a down"

We had a case this weekend where the primary tech put a downed server into
maintenance, but didn't notify anyone, so the 2 other techs on the call list
showed up at the office to check the issue.
