Mike,
I've hit this before too.   Normally there's something wrong with the 
file why it won't stage from tape to disk.  Most cases it's labeled 
"damaged" or needs a samfsck run to correct the problem.    There may be 
other reasons.
I run a 'sfind /cubs -vsn 100858' to locate the remaining files and then 
do a "sls -D" to see what's going on.

In your case it appears to labeled damaged in both copies.  First do 
"undamage -c1 file" and try to stage the data "stage -c1 file".
If this fails you can try samfsck or see if you can manually restore the 
data (star). Lastly, check with the owner of the data in most cases I've 
found the user said it could be simply deleted.
