that the person registering has to know something about the SAM in your case
I have also installed ZBBlock which blocks the worst offenders out there, and
anyone on the stopforumspam database, from even viewing the site!

Stop Forum Spam is pretty good - we use this on a project site I help
with - and it has helped quite a bit. However, the trouble is that a
lot of Spam is actually posted by paid humans - rather than machine
reading ... or by using some auto-spamming tools, so it's very tricky
to keep on top of.
If you need any assistance with killing the spam - I'm sure a few of
us would be willing to help.
