I didn't take a deep look into the current caching talk and into the ways you do the MS-update. But: Do you only download the files or do you do the automated updates via Internet-Explorer? If the last, has anyone tested that these updates are cachable? I could imagine that they are 'individualized' in a way caching does not work.
