I have / had a working upload form but mine was written specifically for
pictures. You might be able to modify it is you want to give it a try.

Ive just received 10 emails from Sambar, saying attachments not allowed
(filters etc..)
is anyone online now - & willing to upload to their own server ?
(virus check first, but it is virus-free)..
i'll be home/online in 4-5 hours & will upload to my own server.
