RMDBeans not cleaned up if Sequence creation fails

If a createSeq comes in an RMD bean is created. If the sending of the
CreateSeqResponse msg fails the RMD bean isn't cleaned up.
I also find that if a duplicate App response message comes into a client on the
back channel of a makeConnection, an error occurs because the AxisService
object can't be found. Setting the AxisService object in the
checkForSyncResponses method cures this and doesn't break any of the Sandesha2
unit tests.
