Remediation involves root cause analysis, troubleshooting and solutioning to bridge the timetable to MnCHOICES 2.0. MnCHOICES is a comprehensive assessment tool and protocol used to assess people who may have long term care needs. MnCHOICES was designed to replace existing paper tools for eligibility and assessments for programs administered by CSA, and the Continuing Care for Older Adults Administration within the Department of Human Services (DHS). MnCHOICES has been implemented statewide for assessments and eligibility since July 2014 and continues to be upgraded with necessary functionality and enhancements. *Other Information Related to the Request:* The current database (MnCHOICES version 1.0) has structural and integrity deficits due to insufficient database design. Meanwhile, MnCHOICES was built with Microsoft Silverlight, which is being deprecated in 2020, requiring a new platform for the application. We will be moving to HTML5.
