When creating an instance, if you are using Database Manager in same machine
as SAPDB is running, just configure, in first step, the database name (don't
put server name, user name and password).

If you are creating a database on a remote computer, in Database Manager
wizard first step you need to put server name (or IP address), user with
rights for creating the database (Administrator in WinXX environment, as
example), and correct password. If you are not using Administrator, the
account used must have logon locally rights. I don't know if have any other
special needs (may be logon as service or logon as batch).

In later step you'll be required to create a DBM and DBA accounts, and
define their passwords.
