I have done this before. The basic things you need to do are:
- Create a multi-ship address that has a FK to the Order model
- Create a MultiShipItem model that ties carts, orders and addresses
together
- Create a view & form to manage multi-shipping recipients
- Create some custom listeners to add context to the templates so you can
tell if multi-shipping is available or being used.
- Also need an order_handler to update the models during checkout

Hopefully that will help point you in the right direction.
