Also check:
1) that the ajax call gets made when you change the country. Use
Firebug, or chrome's dev toolkit for this.
1b) if the ajax call gets made, check the response. It should be an
error, if the change's not happening.
2) if the ajax call doesn't get made and you implemented your own
templates, there's some templates you should be including, which
initialize the javascript needed for those forms to work.
