In my setup I compiled the kernel with the following modules

SCSI Emulation
SCSI Support
SCSI Generic
SCSI CD-ROM
ISO9660

and did not compile IDE-CD support. In /etc/modules.conf I have

alias iso9660 isofs
alias scd0 sr_mod
alias scsi_hostadapter ide-scsi
pre-install sg     modprobe -k ide-scsi
pre-install sr_mod modprobe -k scsi_mod
pre-install scsi-mod modprobe -k ide-scsi
options ide-cd ignore=hdc
