It works ok, with the exception of text, which does get rendered without errors, but is not visible. My only lead to this is that while the correct SalGraphics object is passed to ATSLayout::DrawText() in vcl/aqua/source/gdi/salatslayout.cxx, the actual CGContext inside it is DIFFERENT than what was originally used in the virtual device in canvas/source/cairo/cairo_canvashelper_text.cxx (Surface::createVirtualDevice() in canvas/source/cairo/cairo_cairo.cxx).
