Is it PCIe or AGP? I had some trouble getting AGP 8x to work recently
(due to kernel changes I think).

Neither pci nor agp under /proc/driver/nvidia, only
/cards (0 bytes)
/warnings (0 bytes)
registry (0 bytes)
version (0 bytes)

although in /etc/X11/xorg.conf

Section Device
Identifier  Configured Video Device
  Driver nvidia
EndSection

which driver, however, has no acceleration.

My machine seems to pass all the checks and when I run glxgears, I get
around 8700 FPS

 and without acceleration. This confirms that 3D is not working.
