Turns out my custom /usr/share/X11/xkb/symbols/il
was overwritten, and thus il(katef) was not a valid
variant; the rest of the symptoms follow. kxkb had
nothing to do with it.

The indicator only option of the KDE Keyboard
Layout Switcher has always been used to allow the
keyboard layout to be switched by other means, and
just be indicated in KDE. This is also how I've
used it: I set a layout-switching option in the XKB
configuration, and used it. Now, it breaks: indicator
only means my XKB-set layout-switching shortcut
does not work.

My XKB settings do make it into the session:

$ setxkbmap -print

xkb_compat{ include complete+ledscroll(group_lock)};
xkb_symbols   { include

The switching option is the +group(alt_caps_toggle)
under xkb_symbols -- but alt_caps does nothing
(actually, worse than nothing: it is interpreted
as caps).

BTW: The reason I'm doing this is because I want to
use my own variation on the il layout, which
I named katef. I don't know what that :2 next
to it means. What I put in /etc/default/keyboard
is:

XKBMODEL=pc101
XKBLAYOUT=us,il
XKBVARIANT=,katef
XKBOPTIONS=grp:alt_caps_toggle,grp_led:scroll
