That would depend on how much attenuation the human body provides, and the coupling efficiency to the pacemaker wiring. Both of which would depend on the frequency of the threat. I have done no HPM work, but I expect we are talking about microwave (1 GHz +) threats. I think the attenuation would increase and clearly coupling efficiency would decrease with increasing frequency. 5,000 V/m coupled to a 10 GHz tuned dipole would generate about 25 Volts. I expect that the duration of the transient would be well below 1 microsecond. I don't know any physiology, but I expect that a 1 ns to 1 us transient is much less harmful than 1 ms transient. By the way, a 10 GWatt ERP source generates 5000 V/m at about 30 meters. Using commonly available lab items (and a 100 kV power supply) I should be able to generate 5 kV/m at 3 meters during a 1 ns pulse width. Wonder what this would do to a cell phone?
