Hello,

Sorry I had to leave this for a few days ...
Anyway I believe I have discovered where the problem lies. First things
first, booting with kernel '3.16.0-43-generic' works OK.

The other thing I discovered (latest kernel) is that if my TV tuner is
active the system won't boot. The light on the tuner goes out after the
boot starts and the system hangs. When I power down and restart the
light is out and will come on again after I see the 'ACPI PCC Probe
failed starting version 219' and the system boots. If I disconnect the
tuner from scratch before switching on the system boots OK with the
latest kernel.

This also explains why I noticed that it would boot OK after using
Windows first as it switches the tuner off on shutdown / restart.

So it looks like the problem is with the TV tuner or the driver more
likely, but only with the latest kernel. Its a fairly old tuner - DVICO
usb (AU version) - and all I did to install it was to download
"http://www.linuxtv.org/downloads/firmware/dvb-usb-bluebird-01.fw"; and
copy to /lib/firmware. I use MeTV.

Hopefully this helps. Again, any advice / tips much appreciated.

Just upgraded from 14.10 to 15.04 and now Ubuntu fails to start from
powering up. I select 'ubuntu' on the grub loader screen and then get a blank
screen. Computer just hangs. Have tried Ctrl + Alt + 1 etc, and also leaving
for several minutes but nothing happens.
Once I power down manually and restart and try again I see a message 'ACPI
PCC Probe failed, starting version 219' and PC boots OK.

This has repeated each time from scratch.

The current implementation of Machine Check handler and HMI handler in
Linux, goes down kernel panic path for unrecoverable errors. On FSP
based system FSP also gets notified about these errors which then
forwards it to PRD (that runs on FSP) for error analysis and gard record
creation.

On OpenPower (BMC based system e.g. Habanero from TYAN) where PRD runs
in Linux host, it never gets a chance to do error analysis at the time
of Linux crash and no gard record is created for such errors. Since the
faulty component never gets de-configured, the system is vulnerable to
get hit by same HW error again.

To fix this issue, a new OPAL call 'opal_cec_reboot2()' has been
introduced to trigger a checkstop on BMC based system to inform BMC/OCC
about this error, so that BMC can collect relevant data for error
analysis and decide what component to de-configure before rebooting.
Linux kernel should invoke this opal call for unrecoverable MCE and HMI
instead before calling kernel panic so that OCC is informed about the
error.

The kernel changes has already been posted to upstream and are listed

  The wireless key will make the wifi chip stop working after re-enabled. 
Reproduce rate is > 80%
  (It looks like if there is a wireless connection profile in the list this 
issue won't happen)

  Steps:
  1. Install 14.04.2 (3.16.0-30), boot to desktop
  2. Do not connect to the wireless network, disable the wireless network with 
the hotkey.
  3. Re-enable the wireless network with the hotkey

  Expected result:
  * Wireless network successfully disabled and re-enabled without problem. AP 
available for connect.

  Actual result:
  * Wireless network can't be re-enabled, no AP found in the list.

  I am using Lenovo M30-70.

  I tried many workarounds from
  https://bugs.launchpad.net/ubuntu/+source/linux/+bug/1320070 but no
  change, the connection is lost after a while.

  Finally I tried the rtlwifi_new driver from 
https://bugzilla.kernel.org/show_bug.cgi?id=83641#c1.
  I first thought it works stable but after a while it also loses the 
connection (see my comment there). After reconnect it works for a while again.
  I uninstalled the rtlwifi_new driver for this bug report.

  At least on touch devices BlueZ reports a quoted hostname like
  "Aquaris E4.5 Ubuntu Edition" which is then send over to other devices
  including the quotes.

'"Aquaris E4.5 Ubuntu Edition"' for device 'hci0'
  bluetoothd[6402]: plugins/mgmtops.c:mgmt_set_name() index 0, name "Aquaris 
E4.5 Ubuntu Edition"

  BlueZ reads the hostname from /etc/machine-info on startup which has
  the following set

  PRETTY_HOSTNAME="Aquaris E4.5 Ubuntu Edition"

  Code in BlueZ needs to be changed to ignore the quotes from the value.

Information extracted from dmesg:
Firmware 25.17.12.0

I have modified /etc/modprobe.d/iwlwifi.conf to include 
"options iwlwifi 11n_disable=1 power_save=0" and it really helps but still I 
get Wifi disconnections and poor network stability.

Any news from that issue?

  [Regression Potential]

   * The patches that I backported from 15.04 will preserve the current
  driver behaviour on linux-generic (3.13) and linux-generic-lts-utopic
  (3.16), thanks to the use of specific macros in the code which check
  the kernel version. As a result, no regressions of any kind can be
  expected on these kernels. As a plus, support for kernels up to 3.18
  is also included.

- I did observe multiple times now, that killing the wpa_supplicant + 
reconnecting to the wifi solved the no traffic issue. 
- I can reproduce it quite good in the company  by changing the floor (and 
thus, getting a different access point with the same ssid)

- Does that workaround work for your "no traffic" situations, too? With
having looked into the code, I bet the crypto WPA stuff is done with the
wrong (old, before roaming) BSSID in the keys or challenge response or
whatever.

  The broadcom-wl driver have a few issues that have not been corrected in the 
ubuntu package.
  Supplied with this bugreport is a monolithic patch that addresses some of 
those.

  Summary/testing of patch:
  - 2 days of local testing with associate/disassociate/scan between 2 
access-points. (same model and running same openwrt version, so might not be a 
good test).
  - Suspend/resume seems to work (no extensive testing there yet).
  - Testing been done on a MBP 11,3 with a BCM4360 card running Ubuntu 15.04 
with kernel 4.1.3 (vanilla)
  - Only 4.x changes are new, rest have been in use by several projects 
(ubuntu/gentoo/archlinux etc) for some time.

  
  Feel free to split it up into several patches This have been a locally 
maintained repo that have used several different sources and that's the reason 
for the monolithic patch.

  One issue that is left is the:
  if (WARN_ON(!bss))
  return;
  in net/wireless/sme.c but this does not seem to cause issues.
