First of all, you don't sound dumb at all.  Don't beat yourself up.  We're all 
in this mac community together.  Let me see if I thus can help you out.

Hotspots let you quicly jump to different parts of the screen.  So basically, 
let's look at a practical example which I use them for.  I use ProTools for all 
my audio production work.  In ProTools, one of the things which is essential 
for me to do is to adjust the output level on a given track.  This might not 
make any sense, but it's not relevant.  The bottom line is, the slider which I 
must interact with in order to do this is about dead smacked center of the 
screen.  So, this means trying to go from either the bottom or top of the 
window then vo+left arrow or right arrow respectfully, is quite tedious, as you 
can imagine.  This is where hotspots come in really nice.  I navigated with the 
vo keys normally to the item, then set a hotspot, so that now, with one 
voiceover command, I quicly can jump no matter where I am in the window, to 
that control with only one keyboard shortcut rather than having to move all 
over the place just to find the thing.  

Here is another example where it comes in really really handy.  Another thing 
you can do is to actively monitor hotspots.  So if you have something that 
dynamicly changes, you can actively set a hotspot to that item then have it 
automatically read to you when that area changes in any way.  For example, 
let's take the lcd section of ITunes.  A lot of times, if I'm burning a 
playlist to CD, or say if I'm downloading something from the store that I 
bought, or maybe I'm importing tracks from a CD.  Rather than having to 
periodically look at my lcd section to keep up with the progress, I can set a 
hotspot to that lcd section, then can not only hit one keystroke to then jump 
to it quickly, but now, I also can monitor it actively, so if I do that, then 
each time it updates, say once a track finishes importing and the next one 
starts, rather than me having to constantly sit there babysitting that lcd 
area, I could be down in the songs list table, or out browsing the store etc. 
yet, i
 t'll automatically keep reading to me as the lcd changes until I sztop 
monitoring that hotspot.

Go have a look at the very last heading in chapter 5 of the getting started 
guide.  It will explain things.

Hotspots are places you can connect your laptop or other wireless computer 
to.  So if you create a hotspot with your phone, you would give the wireless 
connection on your phone, and let's say you are on the bus, and you have 
your laptop with you, and you want to connect to the Internet.
You turn on your hotspot with your phone, and you use your laptop to find a 
hotspot, like you would at a coffee shop, or on your router at home, only 
you are connecting to the Internet using your phone's data, via your phone.

It is also important to distinguish here between Personal Hot Spots, which is 
what you are describing, and Hot Spots which are part of VoiceOver.  Hot 
spots allow you to mark places on the screen for later use.  Personal Hot Spots 
do as Glenn describes below.
