The code that caused this problem has been found. It was poppler commit
512b2c654fd80c83b82e7adc828a478a18de17ab on July 26, 2009. You have
poppler 0.12.0 as a dependency, and this commit was made prior to that
release.
I posted a patch against the latest poppler source on freedesktop. Now
Problem is in either libpoppler-glib4 0.12.0-0ubuntu2.1 or libpoppler5
0.12.0-0ubuntu2.1 dependency, not evince. Evince works for sample pdf
when linked against pre-0.11.2 poppler libraries. ePDFView acts in the
same manner, working in earlier libraries, breaking on popper
post-0.11.1
I've reported this bug upstream to the poppler developers at
freedesktop.

It follows in annexe two copies of the same document with other formats. I
think they show well what I can see. The data such as name, residence,
numbers, etc, are lacking. (I apologise for my bad English. I am a
Portuguese.)

Thanks for reporting this bug and any supporting documentation. Since I
can confirm that there is a problem with evince when I open this pdf,
I'm marking it as confirmed and let the developers handle it from here.

viewed it properly in evince without issues. Can you clarify if you're
unable to open it or its not properly displayed
