I selected one polygon (one of the troublemakers, FID:10) from your
shapefile and created a new shapefile with only this polygon. Even with
only one polygon in the layer, you'll still get a label that's placed
incorrectly when using the Over Centroid option . You have two

You are right about the possible placement of centroids outside the
polygon
but as you can see from the screen dump in my original post Qgis is
fully
able to calculate the expected centroids (the brown dots), but ABC
labels
are apparently placed using an other algorithm even if centroid

I tried it with same result.
It seems that only smaller objects have the label way off. Most probably
a bug.

add the polygon centroids and especially for some of the small polygons the
labels are far off (even though these polygons are very regular - they are
made as buffers around a line).
