sudo update-grub gave a strange error that seems maybe related:
/usr/sbin/grub-probe: error: failed to get canonical path of

Here is a new version of the patch.
The patch now properly handles PhpBB3 variant
of Phpass (See comments on stackoverflow for details).
The patch has been tested on Ubuntu 12.04.1.
