I just saw a casper.txt report and I noticed it keeps calling for cd-rom
and I'm using bootable usb. Could that be the problem?

Yes thank you for the information. I tried again and installed using
the entire disk and the system works fine and boots to both Windows and
Ubuntu now. I guess it seems obvious now but the message was not
entirely clear at the time.
