This happened to me when re-generating a PDF from LaTeX that Evince
already had open. The PDF in question has lots of pgf and tikz
graphics. I can't reproduce it reliably.

The attached document triggers the bug from time to time, though I have
not been able to find a way to do it deterministically. Just moving
around and playing with the layout options (dual size, continuous, zoom,
etc.) will eventually make it happen.
