The bug is in mutter and muffin (which is a fork of mutter).

I had the same issue with 12.04 and nvidea drivers. I even downloaded
the Gnome Live image to report it as upstream bug. I think there was a
bug in gnome-screensaver. But now after installing 12.10 the bug
disappeared.

Hi,
I'm affected by this bug, but only when using cinnamon, not unity.
Something was done in compiz to prevent this problem but this was not
done in muffin/mutter.
I've found out that when using xscreensaver the issue happens as well,
BUT when moving the mouse xscreensaver's password prompt shows
This issue has gone away for me. Same for everyone else?
