I've used Vinux. The screen reader launches as soon as you plug in your USB drive and boot your system. The boot image provides 2 options first is to try vinux without installing on your hdd and other is install. I'm not sure but somewhere I read that every linix based OS is accessable from very beginning, There is a keyboard command to launch Orka the screen reader as soon as you boot up for install.
