Hi Arijeet,

It is at the bottom of the laptop, near to the battery pack hinges.

Give me a call if you face any difficulty with locating the tag, will
be easier to guide over the phone.

That depends on the mobility and illness of the patient, assistant is need for 
purchasing medicines in particular.  Although it may not be must but always 
advised/preferred

People with sight do not have that ‘someone’ because of any insistence. It’s 
purely a choice and availability factor. Do note though, forms do require 
contact details of a person. But no hospital has ever insisted on physical 
presence 24 by 7.

And are we then saying that if you don’t have an attendent, you don’t get the 
treatment at all?
