First log into Passport India website by creating your account. Then you may find there a link to apply for fresh/ reissue of passport. After filling the form and checking the list of conditions and declarations you need to submit the form. In the page after logging in you will find a link to view submitted forms and once clicking on it you will be taken to the page which contains your form number followed by a list which would turn into links only after checking your form number. Thereafter you have to click on the fee payment link which would take you to the page where you can select your PSk for appointment and pay the fee online using various options like net banking, debit/credit cards. Once the fee payment and appointment is fixed you have to take a print of the appoint receipt and the fee receipt along with your submitted form and attend the PSK on the appointed date and time slot... w.r.t proofs: your SSLC marksheet is very important and your name is picked as it looks in the SSLC certificate. If aadhaar is there it is enough, but also take with you your PAN card and voter ID in hand along with a set of photocopies of all the docs. Ofcourse your disability certificate also need to be furnished. I provided my State disability certificate along with the E-certificate provided by UDID. w.r.t address proof: what ever the document you provide for address proof will be taken into account and anyhow you need to provide the address as it reflects in your POA in the form while filling. Appropriate Police station is to be mentioned. And ofcourse Police personnel will visit to verify your address. You can get your passport in hand within 10 days from the date of your appointment if everything is clear, so don't go for tatkal unless very urgent as the cost is double...
