We are looking for formats of disability certificates issued by
different state governments in India. So, I request you to take some
strain and spare some time for us in this regard.
I would be grateful if you can type all the particulars, not your
personal details, mentioned on your certificate by taking some
sighted assistance and send the typed documents to me. If you can't
do that, atleast scan your disability certificate and send to me so
that I'll take up that task.
