[AI] list of hospitals regarding disibility cirtificate in kolkata needed.

dear members, as the subject line says i am looking for governement 
recognized centres that have the authority to issue disability 
cirtificate in kolkata.

i have recently shifted to this city. so i have no idea about this.
i have tried my self. for the hospitals. and i came to know that these 
hospitals are divided according to ward no. that means we have to first 
of all find about the area of MLA and then for the further action.
we have done this also, but one of staf member of calcutta medical 
coledge of siyalda branch said that i have to submit the written 
application in bangla. i can't write bangla not only this. any oficial 
work is perform in english. but he refuge to except letter in english.
please suggest what to do. i am living in khidder pore area. and my ward 
no. is 79.

please help me in this matter.
any assistance in this issue will be highly appritiated.

Dear Gufran,

Yes in Kolkata you have to apply for disability certification according to
your ward. As you stay in Ward No 79, assigned hospital for you will be
N.R. S, Medical College. You can call us at 033 24418080 on weekdays
between 11 a.m. and 5 p.m. and we will try to give you details information
and help you in procuring the certificate. Or you can mail us your phone
number and we can talk over phone and give you required information. Our
email id is sruti.d...@gmail.com.

6 years back I had gone through the same process and there was no
issue of language.  I had submitted my application in English with the
residence proof provided by local councilor. The hospital was
Chitranjan hospital in Park Circus.
Some certain days in a week, team of doctors examine applicant for
providing disability certificates.
