RE: [AI] UDID card applied - further steps

Yes, agreed with Ajay.

It is advisable to not to wait for any call from hospital and visit there
along with all your documents.

They may ask you to get checked your eyes again and once you submit your
documents, you will probably receive your UDID card within 15 days.

Thanks & Regards,
Deepak

Second Harish ji, even if one has applied for disability certificate few months
ago, status moves only when you visit the hospital along with your UDID form
and other docs.
Need not wait for the call, one can directly visit.

Cheers,
Ajay

Better to visit once to your district welfare office or district hospital
from where you have been issued disability certificate along with your
aadhar and UDID application and receipt.

Hi

How old is your disability certificate?

Did you apply for UDID along with disability certificate

I had applied for both separately and it worked for me only when I went
to the civil hospital again. Until then, it kept showing the same info
for me.

Wait for some days. your chosen hospital will recieve a
communication/notification in their system, if so, they will call you.
If not. visit your hospital with your UDID application and disability
certificate. as they will upload your disability certificate and verify
your application  you will get e-disability certificate and UDID number
immidiately. hard copy will be sent after some times.

Hi Nehal,

Its good to visit the chosen hospital along with your certificate,
printout of the completed UDID form, and ID proof.
As far as I know, they upload the disability certificate from the
hospital side then only the process gets completed.

HTH,
Ajay
