Hopefully I commented and attached in the right order (the note under Create a
new attachment makes me wonder).

Image Should be shows document opened by Word 97 with MS FileFormatConverter
-- not perfect, but shows the idea.

Bad docx is same doc in OOo 3.0, textboxes all jammed into upper right corner,
as well as both graphics completely transparent in white area of page.

I have had same issue under Win2K and Ubuntu Hardy, with OOo 2.4.1 and 3.0 beta.
 Cell colors vanish when either a) the document background or b) the window
color [3D objects in Windows] are adjusted below the threshold. 
Manually-colored cells and conditional-formatting-colored cells all lose their
color, until background or 3Do color is raised beyond the threshold.

A customizable note indicator would be handy -- whether customizable on a
cell-by-cell basis, doc-by-doc, or just machine-by-machine.  For example, since
I'm 'red-green color-blind', the small red square can be difficult to see
against certain fill colors.  The ability to change the size and/or shape and/or
color of the note indicator would be sweet.
